WellFirst Health Facebook Follower Giveaway Official Rules

ELIGIBILITY: NO PURCHASE NECESSARY TO ENTER OR WIN. Purchasing products will not improve your chances of winning. The WellFirst Health Facebook Follower Giveaway (the Sweepstakes) is only open to current employees of SSM Health Care Corporation (the “Sponsor”). All participants must be United States citizens age 18 years or older to enter and win. Any determination regarding the type of participant’s employment status is made solely by Sponsor. The Sweepstakes is subject to all federal, state and local laws and regulations and shall be governed by Wisconsin law. The Sweepstakes is void wherever prohibited or restricted by law. By entering the Sweepstakes, entrants: (i) agree to accept and be bound by all terms of these Official Rules and (ii) agree that Wisconsin courts have jurisdiction over any dispute or litigation arising from or relating to the Sweepstakes and that venue shall only be Madison, Wisconsin.

HOW TO ENTER: Beginning November 12, 2019, visit https://www.facebook.com/WellFirstHealth/ and “Like” the WellFirst Facebook page. Sponsor, in its sole discretion, shall select one (1) winning entry at random from eligible participant entries after the close of the Sweepstakes Entry Period (as defined below). The Winner shall receive an Apple® iPad 7^th Generation 10.2” Tablet, 32 GB, WiFi, in Silver, subject to terms and conditions of these Official Rules. The Sweepstakes begins on November 12, 2019 at 12:00 a.m. Central Standard Time and ends on December 15, 2019 at 11:59 p.m. Central Standard Time (the “Sweepstakes Entry Period”).

One entry per person. Any duplicate entries shall be deemed void. All entries will become the property of Sponsor and will not be acknowledged or returned. No mechanical reproductions, mailed entries, facsimiles, mass submissions, or submissions generated by a script or macro will be accepted by Sponsor and all such entries will be deemed void. Creating multiple Facebook accounts to enter the Sweepstakes multiple times will render the participant ineligible from the Sweepstakes. Sponsor is not responsible for misdirected, inaccurate, incomplete, lost, late, delayed, undelivered, damaged, or illegible entries, and all such entries will be deemed void. Sponsor is also not responsible for technical, hardware, or software failures of any kind, or other errors or problems that may occur in connection with or relating to the Sweepstakes, whether computer, network, technical, mechanical, typographical, human, or otherwise, including, without limitation, errors or problems that may occur in connection with the administration of the Sweepstakes, the processing of entries, the announcement of the prizes, in any Sweepstakes-related materials or that may limit prize fulfillment or a participant's ability to enter the Sweepstakes. Sponsor reserves the right, in its sole discretion, to cancel, modify, or prematurely conclude the Sweepstakes should any computer virus, malfunction, fraud, tampering, unauthorized intervention, or other cause beyond its control corrupt the administration, security, or proper operation of the Sweepstakes.

CONDUCT: By entering this Sweepstakes, entrants agree to be bound by these Official Rules. Failure to comply with these Official Rules may result in disqualification. Sponsor reserves the right at its sole discretion to disqualify any individual found to be tampering with the entry process or the operation of the Sweepstakes or the Website; or to be acting in any manner deemed by Sponsor to be in violation of the Official Rules; or to be acting in any manner deemed by Sponsor to be unsportsmanlike or disruptive, or with intent to annoy, abuse, threaten or harass any other person. False or deceptive entries or acts will render the participant ineligible. Any attempt by an individual to deliberately damage any web site or undermine the legitimate operation of the Sweepstakes may be in violation of criminal and civil laws. Should such an attempt be made, Sponsor reserves the right to seek remedies and damages (including attorneys' fees) from any such participant to the fullest extent of the law, including criminal prosecution.

ODDS OF WINNING: The odds of winning depend upon the number of eligible entries received.

HOW TO WIN: One (1) winner will be chosen by Sponsor after the close of the Sweepstakes Entry Period. The decisions and determinations of Sponsor will be final and binding. Sponsor will notify the winner by email of their winning entry on the business day following Sponsor’s decision and determination regarding such winning entry. Sponsor reserves the right to request additional information from potential winner to prove eligibility. Within two (2) days of notification of the need for such additional information, potential winners must verify eligibility or another entry will be randomly selected to become an alternate winner.

PRIZES: One (1) winner for one (1) Apple® iPad 7^th Generation 10.2” Tablet, 32 GB, WiFi, in Silver, with a value of $329 to one winner chosen by Sponsor after the close of the Sweepstakes Entry Period. No substitution of prizes or cash will be accommodated or permitted. No transfer of prizes will be permitted. Sponsor reserves the right to substitute a prize of equal or comparable value in the event a described prize or any portion thereof is unavailable for any reason whatsoever. Winners are solely responsible for the reporting and payment of all federal, state and local taxes incurred by acceptance and use of the prize (or any portion thereof), if applicable.

GENERAL RULES AND REGULATIONS: By entering this Sweepstakes, entrants (i) agree to be bound by the decisions of Sponsor, which are final and binding in all matters, (ii) grant Sponsor and its designees the right, unless prohibited by law, to use their names, company name, cities of residence, voices, pictures, biographical information, and likenesses, without compensation, notification, or permission, for the purpose of advertising, information or publicity in any and all manner or media, now or hereafter known, throughout the world in perpetuity, (iii) represent and warrant that they are not participating in the Sweepstakes in the capacity of a reporter and further represent and warrant that they will not write, blog about, publish, or cause to be published any article or book about their participation therein. In order to claim the prize, winners: (i) must meet all eligibility requirements, (ii) may be required to produce proof of identity, address, and birth date, and (iii) must execute and return certain winner documents (as applicable), including an affidavit of eligibility, tax acknowledgment form, liability release and, use of name and likeness releases. Such documents must be completed in full and returned within three (3) days of issuance either via facsimile, U.S. registered mail or e-mail. Failure to do so or if (i) any prize or prize notification is returned as undeliverable, (ii) winner declines his or her prize, or (iii) a potential winner is determined to be ineligible for any reason will result in disqualification of such winner and an alternate winner may be selected by the Sponsor. Sponsor will use reasonable efforts to pick an alternate winner, but in the event an alternate winner cannot be determined, the prize will not be awarded, or Sponsor may award the prize to another eligible entrant. Official Rules for drawing may be amended by Sponsor at any time. The decision of Sponsor shall be final on all matters concerning the Sweepstakes.

Sponsor and its respective employees, officers, directors, agents, subsidiaries, advertising or promotion agencies, and affiliated entities expressly disclaim any responsibility, and participants agree to hold Sponsor and its respective officers, directors, agents, subsidiaries, advertising or promotion agencies, and affiliated entities harmless from liability for any injury, disability, or loss to any person or property (including death) relating to participation in this Sweepstakes (including the manner in which the winner is selected and confirmed), or the delivery and/or subsequent acceptance, use, or misuse of any of the prizes awarded and claims based on publicity rights, defamation, or invasion of privacy. Neither Sponsor nor its respective employees, officers, directors, agents, subsidiaries, advertising or promotion agencies, and affiliated entities are responsible for: (i) any loss (financial or otherwise), liability, injury (including death), or damage to persons or property that may be caused directly or indirectly, in whole or in part, by the use or misuse of any prize items or services provided, or (ii) any loss (financial or otherwise), injury, or damage to persons or property that may be caused directly or indirectly, in whole or in part, from downloading any material from the Web Site, regardless of whether the material was prepared by Sponsor or a third party, and regardless of whether the material is connected to the Web Site by a hypertext link. Sponsor and its respective employees, officers, directors, agents, subsidiaries, advertising or promotion agencies, and affiliated entities further assume no liability either for the cancellation, modification, or premature conclusion of the Sweepstakes for any reason or through the acts or defaults of any entity or person providing any prize. All such losses or expenses must be borne by the affected participants and/or winner.

LIMITATIONS OF LIABILITY: If, for any reason, the Sweepstakes is not capable of running as planned for reasons that may include, without limitation, infection by computer virus, tampering, unauthorized intervention, fraud, technical failures, or any other causes that may corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, then Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend the Sweepstakes in whole or in part.
